VisaScreen Certificate for Occupational and Physical Therapists: The Credential Before Your H-1B or Green Card

Your H-1B or green card can be approved and still stall at the visa or admission step if your VisaScreen certificate is missing. Here is how it works.

If you are a physical therapist or occupational therapist hoping for an H-1B or a green card, there is a credential that sits between your degree and your visa: the VisaScreen certificate. Issued by CGFNS International, VisaScreen verifies that your education, licensure and English proficiency meet US standards, and federal law generally requires it (or an equivalent certificate) when a covered healthcare worker gets a work visa, is admitted, changes status or adjusts to permanent residence. Your employer can do everything right and your case can still stall at the stamping window or the I-485 if the certificate is missing.

Below: what VisaScreen is, where it appears in each visa path, how it differs from the NPTE, NBCOT and your state license, and how to sequence it.

Status line, as of 2026-10-14. In effect: the VisaScreen requirement (INA 212(a)(5)(C), 8 CFR 212.15) for therapists; Proclamation 11069 extends the $100,000 H-1B entry restriction to 2027-09-21, but the fee is blocked by two federal courts (California v. Mullin and Global Nurse Force v. Trump) and is not being collected. Proposed only: a $103,265 cap-subject H-1B fee (comments closed 2026-09-24, no final rule) and elimination of the 60-day worker grace period (comments close 2026-11-10). The October 2026 Visa Bulletin is published; the November bulletin is not.

What a VisaScreen certificate actually is

The legal hook is Section 212(a)(5)(C) of the Immigration and Nationality Act, added in 1996, and its regulation at 8 CFR 212.15. It says certain healthcare workers, other than physicians, who come to the United States to work in their profession must present a certificate from a credentialing organization approved by DHS. The covered occupations, regardless of where you trained, are registered, licensed practical and licensed vocational nurses, occupational therapists, physical therapists, speech-language pathologists and audiologists, clinical laboratory scientists and technicians, and physician assistants.

Before issuing a certificate, the organization must verify four things:

  1. Comparability: your education, training, license and experience match what a US worker in the same profession needs.
  2. Authenticity: those records are genuine, and your license is unencumbered.
  3. An exam: you passed the occupation's licensing or certification exam, or a recognized test that predicts success on it.
  4. English proficiency, through an approved test unless you qualify for an exemption.

CGFNS VisaScreen is run by CGFNS International through its International Commission on Healthcare Professions, and the regulation authorizes CGFNS for every covered occupation. It also authorizes two occupation-specific bodies: the Foreign Credentialing Commission on Physical Therapy (FCCPT) for PT and NBCOT for OT. Confirm with your employer's attorney which one fits your case before you pay for an application.

The certificate has a shelf life. It must be used for an admission, change of status or adjustment within 5 years of its issue date, and a nonimmigrant presents it again at each visa issuance and admission. Check the issue date on yours against your expected stamping or I-485 timeline.

VisaScreen vs. NPTE, NBCOT and your state license

These credentials are often treated as one thing. They are not.

CredentialWho issues itWhat it is forNeeded for immigration?Needed to practice?
VisaScreen certificateCGFNS International (or another approved body)Proves education, license and English meet US standards for immigration purposesYes, for covered occupationsNo
Foreign credential evaluation (PT)FCCPT or another evaluator your state board acceptsLets a state board decide if a foreign PT degree is equivalentIndirectlyUsually, for foreign-educated PTs
NPTEFederation of State Boards of Physical Therapy (FSBPT)National PT licensing examIndirectly (can meet the certificate's exam element)Yes
NBCOT certification examNational Board for Certification in Occupational TherapyOT certification (OTR) used by state licensingIndirectly (can meet the certificate's exam element)Yes, in practice
State licenseEach state's PT or OT boardLegal right to practice in that stateOften needed for the H-1B petitionYes

Two consequences follow. Passing the NPTE or NBCOT exam does not by itself give you a VisaScreen certificate, and the regulation says the certificate does not authorize you to practice. And the same transcript may need to go separately to CGFNS, an evaluator and a state board, each directly from the school. If you trained abroad, the licensing side has its own clock; our guide to moving a PT or OT license between states covers that half of the problem.

Where VisaScreen shows up in each visa path

The certificate is linked to immigration steps, not to your job offer. Here is where it typically appears for therapists, as of October 2026.

PathWhen VisaScreen typically comes upOther gate to watch
F-1 OPT (12 months)Not part of the I-765; the rule excludes training within an F or J programState license or graduate permit to practice; OPT's 90-day unemployment limit
Cap-exempt H-1B (university hospital, affiliated nonprofit)Before change of status, consular stamping or admissionState license; LCA
Cap-subject H-1B (lottery)Same, after selection and petition approvalLottery; $100,000 fee is extended on paper but blocked in court

Practice varies on when the certificate is submitted; your own notice and your attorney govern over this chart.

Physical therapist H-1B: what has to line up

H-1B is a specialty-occupation visa. US entry-level PT practice requires a Doctor of Physical Therapy and OT practice a master's or OTD, which supports that classification. Assistant roles (PTA, OTA) are associate-degree jobs and generally do not qualify.

The practical stack for a physical therapist H-1B is:

Demand is real. In DOL's FY2026 LCA selected statistics through Q3, occupational therapists were 5.0% and physical therapists 4.7% of 830,710 certified LCA positions. Only software developers (29.1%) and electronics engineers (7.5%) had larger shares. That counts certified LCA positions, not H-1B approvals or hires, so it shows employer intent, not your odds. In the BLS September 2026 jobs report, health care added 17,000 of the 29,000 payroll jobs, with unemployment at 4.2%.

Cap-exempt employers matter a lot here. University hospitals and affiliated nonprofits can sponsor outside the lottery; our guide to cap-exempt healthcare and university hospital H-1B employers explains who qualifies. On the $100,000 fee, the USCIS H-1B page says DHS is complying with the court order and still plans to collect the fee if the order is lifted. Do not plan around the fee being either collected or permanently gone.

Occupational therapist visa sponsorship: the PERM difference

OT and PT look alike until the green card. Physical therapists are on Schedule A, Group I under 20 CFR Part 656. That lets an employer file the I-140 directly with USCIS with an uncertified labor certification form, skipping the PERM recruitment test. A PT needs either a permanent state license in the state of intended employment or a letter from that state's licensing official saying the PT is qualified to take the licensing exam.

Occupational therapists are not on Schedule A. Some older guides say they are; the regulation lists physical therapists and professional nurses. So occupational therapist visa sponsorship for a green card normally runs through full PERM. That timing difference is big as of October 2026, per DOL FLAG processing times:

The Schedule A details and their own pitfalls are in our Schedule A shortage occupations guide. For the full OT picture, including H-1B and TN, see occupational therapist visa sponsorship, and for PT and other allied health roles see physical therapist and allied health visa sponsorship.

Visa Bulletin: where EB-3 therapists stand, October 2026

Therapist green cards are filed in EB-2 or EB-3 depending on the job's requirements. EB-3 Final Action Dates from the October 2026 bulletin:

The November 2026 bulletin had not been published as of 2026-10-14, so any "November cutoff" you see is a forecast. Check the live Visa Bulletin on travel.state.gov. Long waits matter because the certificate must be used within 5 years of its issue date.

How to apply for a CGFNS VisaScreen certificate

  1. Confirm the right credentialing organization for your occupation with your employer's attorney (CGFNS VisaScreen, or an occupation-specific body such as FCCPT for PT or NBCOT for OT).
  2. Open your application on the CGFNS website and select your occupation. Use your name exactly as it appears on your passport, and note any name differences across your records.
  3. Request education records sent directly from your school on the CGFNS forms. Foreign universities can be slow.
  4. Request license verification from each licensing authority where you hold or held a license, sent directly to the credentialing organization.
  5. Take an English test the organization accepts for your occupation and have scores sent directly, unless you are exempt. The regulation exempts graduates of schools in Australia, Canada (except Quebec), Ireland, New Zealand, the United Kingdom and the United States.
  6. Respond quickly to deficiency requests. Missing course hours, unclear clinical training or a name mismatch can pause the review.
  7. Receive the certificate and give a copy to your attorney. Keep the original safe for your visa interview or I-485.

Take current fees and processing times from CGFNS directly, not from forums.

Sequencing around consular stamping

If your path includes stamping abroad, build in slack. Interview waivers ("dropbox") are gone for H-1B, H-4, L-1, F-1 and J-1; you apply in your country of nationality or legal residence (third-country stamping ended 2025-09-06); and domestic H-1B renewal has not come back. Check the live travel.state.gov wait-times page. Proclamation 10998's 39-country travel restrictions remain in effect, unchanged since 2026-01-01.

Common mistakes

Frequently asked questions

Do physical therapists need a VisaScreen certificate for an H-1B?

Generally yes. INA 212(a)(5)(C) and 8 CFR 212.15 require a healthcare worker certificate, and CGFNS VisaScreen is the best-known one. It attaches to the visa, admission and status steps, not the lottery.

Do I need VisaScreen if I studied OT or PT in the United States?

Usually yes. The rule applies regardless of where you trained. Graduates of CAPTE-accredited PT or ACOTE-accredited OT programs skip the comparability review and English test under 8 CFR 212.15(i), but still need the certificate. Confirm with CGFNS and a licensed immigration attorney.

Is VisaScreen the same as a state license or the NPTE or NBCOT exam?

No. VisaScreen is an immigration credential and does not authorize practice; the NPTE, NBCOT exam and state license govern the right to practice. Most foreign-trained therapists need all of them.

Is occupational therapy a Schedule A occupation like physical therapy?

No. Schedule A at 20 CFR 656.5 names physical therapists and professional nurses, so an OT green card normally goes through full PERM.

Do I need VisaScreen to work on OPT as a new OT or PT graduate?

It is not part of the OPT I-765, and the rule excludes training within an F or J program. It matters at the work-visa or green card step. Ask your DSO and an attorney how it applies to your OPT job.

If you are an OT or PT on F-1 or OPT and want help finding employers that actually sponsor therapists, including cap-exempt hospital systems, talk to F1Jobs. We help with the job search; your attorney handles the filings.

Frequently asked questions

Do physical therapists need a VisaScreen certificate for an H-1B

Generally yes. Section 212(a)(5)(C) of the Immigration and Nationality Act and the regulation at 8 CFR 212.15 require physical and occupational therapists coming to work in the United States to hold a healthcare worker certificate from an approved credentialing organization, and the CGFNS VisaScreen certificate is the best-known one. It is tied to the visa, admission and status steps rather than to the H-1B lottery, so ask your employer's immigration attorney exactly when your case needs it.

Do I need VisaScreen if I studied OT or PT in the United States

Usually yes. 8 CFR 212.15(c) applies to covered therapists regardless of where they trained. A US degree changes the review, not the requirement. Graduates of a CAPTE-accredited PT program or an ACOTE-accredited OT program are exempt from the educational comparability review and the English test under 8 CFR 212.15(i), but the credentialing organization still verifies the license and issues a certificate. Confirm your own situation with CGFNS and a licensed immigration attorney.

Is VisaScreen the same as a state license or the NPTE or NBCOT exam

No. VisaScreen is an immigration credential that USCIS and consular officers look for, and the regulation says it does not authorize you to practice. The NPTE from FSBPT and the NBCOT certification exam lead to the right to practice, and the state licensing board issues the license itself. Most foreign-trained therapists need all of them, and they run on separate tracks with separate document requests.

Is occupational therapy a Schedule A occupation like physical therapy

No. The Department of Labor's Schedule A list at 20 CFR 656.5 names physical therapists and professional nurses. Occupational therapists are not on it, so an OT green card sponsored by an employer normally goes through full PERM labor certification, while a PT green card can use the Schedule A route that skips the PERM recruitment test.

Do I need VisaScreen to work on OPT as a new OT or PT graduate

VisaScreen is not part of the OPT application on Form I-765, and 8 CFR 212.15(b)(3) excludes people receiving training as part of an F or J program. The certificate becomes relevant when you move into a work visa status or apply for a green card, so many graduates start the file during OPT. Ask your DSO and a licensed immigration attorney how the rule applies to your OPT job.
